Application for alteration or addition to the name of a child after registration of birth

Who may apply

	After registration of birth, the parent may apply at any of the births registries to alter or add to the name of a child if desired.
	No alteration/addition to the name of a child shall be made after the child has reached the age of 11.

How to apply

In applying for such alteration/addition of name, the parent must attend in person to make a declaration and produce the child's birth certificate.

Processing time

A certified copy of birth entry with such alteration/addition of name can be issued in 9 working days.

Fees

	No fee is payable if the alteration/addition of a child's name is made within the period of 42 days from the date of birth.
	A fee will be charged if such alteration/addition is made after the period of 42 days from the date of birth.
	A fee is also payable for a certified copy of entry of such alteration/addition.
