Certificate of Registered Particulars

Under the Registration of Persons Ordinance, the identity card holder may make a written request to apply for a Certificate of Registered Particulars to certify the particulars such as name, residential address, date of birth, place of birth, marital status and name of spouse (if any) that are furnished to the registration officer upon his/her application for an identity card.

Application in person

An applicant should bring his/her Hong Kong identity card and a duly completed and signed application form for a Certificate of Registered Particulars (ROP 122) and apply in person at any Registration of Persons office. The fee for the certificate is HK$385.


Applicant who is unable to apply in person owing to health reasons

If the applicant is unable to apply in person owing to health reasons, he/she may make an application by post to the Registration of Persons General & Statistics Unit at 12/F, Immigration Tower, 7 Gloucester Road, Wan Chai, Hong Kong enclosing the following documents:

* a duly completed and signed application form for a Certificate of Registered Particulars (ROP 122);
* a copy of the applicant's and the authorised person's identity card;
* a letter stating the reason for application and authorising a representative to handle the application and collect the Certificate of Registered Particulars;
* the letter must contain the names and Hong Kong identity card numbers of both the applicant and the authorised person, and the contact address and telephone number of the authorised person;
* the letter has to be signed by the applicant and affixed with his/her clear and complete left or right thumb-print*; and
* a certificate issued by a Hong Kong registered medical practitioner on the present health condition of the applicant.
The fee for the certificate is HK$385. Payment may be made upon issuance of the Certificate.
* Holders of Certificate of Exemption who have not applied for a smart identity card before should provide clear and complete left thumb-print.


Applicant who is living outside Hong Kong

If the applicant is living outside Hong Kong, he/she may make an application by post to the "Registration of Persons General & Statistics Unit at 12/F, Immigration Tower, 7 Gloucester Road, Wan Chai, Hong Kong" enclosing the following documents:

* a duly completed and signed application form for a Certificate of Registered Particulars (ROP 122);
* a written and signed request made by the applicant. The request must contain the reason for the application, the name and Hong Kong identity card number of the applicant. If a representative is appointed to collect the CRP, please specify the name, Hong Kong identity card number, contact address and telephone number of the representative on the written request; and
* the request should be affixed with a photograph (size must not be larger than 60x50 mm or smaller than 50x40 mm) and the left or right thumb-print* of the applicant. Both the photograph and thumb-print must be properly authenticated by a Notary Public and clear enough for verification.
  * Applicants who have not applied for a smart identity card before should provide their clear and complete left thumb-print.
  The fee for the certificate is HK$385. Payment may be made by a crossed cheque payable to 'The Government of the Hong Kong Special Administrative Region'. If the cheque is not drawn on a bank in Hong Kong or not in Hong Kong currency, a bank handling charge of HK$60 should be included for each cheque.
