How to make an application

Application for verification of eligibility for permanent identity card can be made by completing an application form ROP 145.

You may also deposit the application into the drop-in box provided at the Right of Abode Section. When you send in the application by post or deposit the application into the drop-in box, you may use the self-addressed envelope.

Please do not send in original documents. All photostat copies submitted will not be returned. Please also send the mailing label after filling in your name and mailing address.

You will be notified in writing to produce the original documents for verification.
