For applicants aged 18 or over |

an application form for Hong Kong Seaman's Identity Book (ID94) duly completed and signed; |

2 identical recent photographs (please see photograph requirement); |
| |

Hong Kong identity card; and |
| |
 |
Hong Kong Seaman's Employment Registration Book. |

B. |
For applicants under 18 |
| |
 |
documents listed in paragraph A above ; |
| |
 |
a parent or legal guardian should complete and sign section 6 to confirm his/her consent to the application; and |
| |
 |
the consenting parent or legal guardian's identity card or valid travel document; and |
| |
 |
the birth certificate of the applicant to prove the relationship between the consenting parent and the applicant; or a court order, if appropriate, to prove that the consenting legal guardian has custodial rights in respect of the applicant. |

[If applicant is married, his/her marriage certificate is required instead of the above mentioned parental consent.] |

Photostat of the documents may be required. |

If applicant is requesting a replacement Seaman's Identity Book, his/her previous Seaman's Identity Book should be produced for cancellation upon collection of the new Seaman's Identity Book. |

If applicant's previous Seaman's Identity Book is lost, damaged or not available, the applicant should provide the circumstances of loss / damage or reasons for its unavailability in Form ID645. A new Seaman's Identity Book will be issued only after extensive enquiries. |
