How to submit an application made outside HKSAR

Application forms are downloadable at this website and also obtainable from Chinese diplomatic and consular missions or the HKSAR Immigration Offices.

Through Chinese diplomatic and consular missions

Applicant may submit the application by post or in person to the nearest Chinese diplomatic and consular missions. The application will be forwarded to the Travel Documents and Nationality (Application) Section of the HKSAR Immigration Department. The prepared HKSAR Seaman's Indentity Book will be sent back to the Chinese diplomatic and consular missions for issue.

Direct application to the HKSAR Immigration Department

Applicant may also submit the application by sending the completed application form, together with the photostat copies of the supporting documents and the required fee directly to the Travel Documents and Nationality (Application) Section of HKSAR Immigration Department. Applicant is required to collect his/her Seaman's Identity Book in person at the Travel Documents and Nationality (Application) Section of HKSAR Immigration Department. Upon collection, the applicant has to produce the originals of the supporting documents for verification.

Documents to be enclosed with the application

A.	For applicants aged 18 or over
		an application form for Hong Kong Seaman's Identity Book (ID 94) duly completed and signed;
		photostat copy of Hong Kong identity card (only the side with the holder's personal particulars and photograph is required);
		photostat copy of Hong Kong Seaman's Employment Registration Book;
		three identical recent photographs (please see Photograph Requirement); and
		a completed fee payment form (ID856) together with a cheque or bank draft made payable to "The Government of the Hong Kong Special Administrative Region" in HK currency or US dollar.The fee is payable at the time of application. The collection of fee does not constitute any assurance that the Seaman's Identity Book applied for will be issued.
B.	For applicants under aged 18
		documents listed in paragraph A above;
		a parent or legal guardian should complete and sign section 6 to confirm his/her consent to the application;
		photostat copy of the consenting parent or legal guardian's identity card or travel document; and
		photostat copies of your birth certificate to prove the relationship between the consenting parent and you, or a court order, if appropriate, to prove that the consenting legal guardian has custodial rights in respect of you.

The photographs should be taken full face, without a hat. The background should be plain. The size must not be larger than 55 x 45 mm or smaller than 50 x 40 mm. The photographic paper used should be of the normal thin type, unglazed on the back and unmounted. Photographs from slot machine can be accepted only if the paper is not of plastic synthetic material and can be firmly affixed to the travel documents.
