| This form is applicable to entry permit application from Chinese residents of the region of Taiwan who do not possess any acceptable travel document to the Hong Kong Special Administrative Region for purposes other than visit. Please read the Guidance Notes carefully before completing the form.
Important:
Completed application form should be returned to the airline office together with other supporting documents for application;
To ensure smooth processing of applications, the application forms should bear the reference number allotted by the airline office. You should clarify with the airline office in case the reference number is missing.
