| This form is applicable to entry permit application from Chinese residents living in Taiwan who do not possess any travel documents which are acceptable to the HKSAR for the purpose of visiting Hong Kong (one entry). Please read the notes for guidance carefully before completing the form.
Important:
Completed application form should be returned to the airline office together with other supporting documents for application;
To ensure smooth processing of applications, the application forms should bear the reference number allotted by the airline office. You should clarify with the airline office in case the reference number is missing.
