This form is applicable to minors under the age of 11 years applying for
(i) a permanent identity card in connection with the application for a Hong Kong Special Administrative Region (HKSAR) passport [Please read ID(E)842A and ROP (E)149 before completing this form.]; or
(ii) a replacement card due to its loss, defacement, damage or alteration of registered particulars.
Note : This form is NOT APPLICABLE to minors under 11 who are going to apply for replacement of smart identity card at the Smart Identity Card Centre under the territory-wide Replacement Exercise. |
