This form is applicable to minors under the age of 11 years applying for

(i) a permanent identity card in connection with the application for a Hong Kong Special Administrative Region (HKSAR) passport [Please read ID(E)842A and ROP(E)149 before completing this form.]; or

(ii) a replacement card due to its loss, defacement, damage or alteration of registered particulars.

  1. For application in connection with HKSAR passport application, the application form should be submitted together with the HKSAR passport application form ID 842. Please refer to the contents of Guidance Notes ID(E) 842A.
  2. Not applicable to card replacement due to loss, defacement, damage or alteration of registered particulars.
In Person To:
  1. For application in connection with HKSAR passport application, the application form should be submitted together with the HKSAR passport application form ID 842. Please refer to the contents of Guidance Notes ID(E) 842A.
  2. For card replacement due to loss, defacement, damage or alteration of registered particulars, application should be submitted in person to any one of the Registration of Persons office.
